Listed below are mandatory items that must be completed and received by Veteran Mentors no later than 1 week before the program commences. All waivers are to be uploaded using the webform at the bottom of this page. Please email all other required information such as dietary requirements and flight details to info@veteranmnetors.com.au.
- Payment finalised
- Copy of Parent’s ID uploaded below
- Waivers signed and uploaded below:
- Planet Commando
- Gold Coast Sky Dive
- Veteran Mentors
- GP clearance form signed and returned
- Flights to Gold Coast airport booked and details provided
- List of dietary requirements or allergies provided
- Medication and non prescription vitamins etc. stored in Webster pack along with child’s photo ID
- Skydiving information provided via survey HERE
